Initial Evaluation

Upon your arrival to the office you will be asked to provide some basic demographic information. If you would like to complete this information prior to your visit to expedite the registration process, you may print and complete the form below and bring it with you to the office.  You may also complete the form online.

Please try to avoid make-up on your face, neck, or upper body so a complete evaluation can be done.

Our RN/ Esthetician will take a complete history of your past skin care and treatments. She will analyze your skin and review treatment options. A physician is in the office for additional evaluation if needed.

At the completion of your first visit you will know the treatments if needed, the skin care recommendation, and possible care products which will allow you achieve the healthy, youthful look and feeling.
